If this count is per milliliter, it would be very good as a number.

But the values you are providing are lacking lots of information: count (total or per ml), motility, forms ...

All those, in addition to many points in history and physical examination are needed before deciding for any management.

Furthermore, if all the values in the sperm analysis are normal, operating for a varicocele would be without any benefit.
