Sometimes I have a very severe pain in my chest when I try to swallow anything even water, or when I breath (usually in the middle)

This is called dysphagia, that is, trouble during swallowing. The source of the pain is the esophagus. Possible causes include:
1. Candida infection: candida is a fungus that can cause infection in the oral cavity and esophagus. Think of it if u see white streaks/patches in ur mouth or throat.
2. Achalasia: trouble in the esophageal motility (the esophagus contracts thru several phases of the swallowing process). This usually produces odynophagia (dyn means pain, phagia means eating/swallowing). It usually starts with pain when swallowing solids & progresses to pain even upon swallowing liquid. Sometimes however it could be only when swallowing liquid.
3. Reflux: when the stomach acidic content travels up to the esophagus due to malfunction of the ring that joins the esophagus to the stomach, the esophagus gets inflamed (esophagitis) , hence the pain.
It's necessary that you seek the advice of a specialist to run the appropriate tests in order to reach the correct diagnosis and treat accordingly.
