My mother 58 y.o had a blood test :LDL-c:189; HDL-c:104 . Does she need a medical trt or just diet? Not diabetic , 1-2 cig/day,her dad died at 63 MI.

What is her total cholesterol? What is her blood pressure ? Is she on blood pressure medications? Current guidelines recommend high intensity statin therapy ( ie Lipitor 40-80 or Crestor 20-40) for all patients who have LDL of 190 or over. This also applies to patients who have known heart or vascular disease and those whose estimated 10 year risk for a cardiovascular event is over 7.5%. Moderate intensity statin therapy ( Lipitor 20 or Crestor 10 ) is recommended for those whose risk is 5-7.5% and for those who are diabetic and their estimated risk is below 7.5%. You can use one of many apps to calculate the risk ( try ASCVD risk or ASCVD Suite). As your mom's LDL is close to 190, she may benefit from treatment.
