This may be physiological gastro-esophageal reflux (GER), which is normal and expected in infants especially in the first 6 months of life. GER results from the fact that the lower esophageal sphincter is still immature; the lower esophageal sphincter is a ring of muscles that joins the lower end of the esophagus to the entrance of the stomach and serves to prevent the return of the stomach content backward into the esophagus. Babies tend to have a loose lower esophageal sphincter, and this improves with age. Because of the return of the acidic stomach content into the esophagus, this results in irritation and at times inflammation of the esophagus, and therefore swallowing becomes painful, so the baby cries especially upon burping.The usual management of GER is by correct positioning of the infant during nursing, regular burping after each and every meal, avoiding over-feeding your baby (a lot of new moms over-feed their infants without realizing it), thickening of the formula with rice water, and/or shifting to anti-reflux milk formula. The main parameter that we rely upon to assess the severity of GER in an infant is the adequacy of his/her growth (how well he/she is adding in weight and length on the growth curve).Of course such diagnosis cannot be established without more thorough evaluation of the child in the pediatrician's office - I strongly recommend that you have your baby seen by his pediatrician who can further assess this milk spitting issue and monitor his weight gain regularly.
