Polycystic ovary syndrome (PCOS) is characterized by increased synthesis and secretion of androgen (male type hormones) by the ovaries. This will stimulate the skin sebaceous glands (that secrete oil) to produce more oil, which will render the skin a more favorable medium for bacteria to grow, causing acne and comedons (black heads). The optimal and radical solution toacne in a wwoman with PCOS is to treat the underlying cause, which is excess androgen. The 1st choice of therapy is combined oral contraceptive pills (OCPs) which will lower androgen production by blocking steroid secretion by the ovaries, hence improving acne. If OCP are not effective, Aldactone (spironolactone), which acts directly on the androgens and lowers their levels, is added to the treatment regimen. Metformin may also contribute but not as much as OCP and spironolactone. RoAccutane (isotretinoin) does not seem to have a major role in treating pcos related acne ; it also places further burden on the liver (increases liver enzymes) which we don't want if you'reaalready on Metformin (which also increases liver enzymes. RoAccutane is also known to alter cholesterol levels in the blood - a side effect that we definitely want to avoid in pcos (because pcos carries the risk of developing high blood pressure and diabetes in the future, so we certainly don't want high cholesterol as well).
