It could be either:
1- new onset juvenile rheumatoid arthritis (a disease in which the immune system attacks the body's own tissues in the joints). It's usually associated with morning joint stiffness (jelling) that improves with moving the joint, redness and swelling, or limitation in the ability to use the affected joint while holding it inthe least uncomfortable position. Spikes of fever occur almost at the same time daily. A single joint, or more than one joint, may be involved. Rash that comes and goes may be noted. Blood studies show elevated increased white blood cells, high CRP and ESR. Rheumatoid factor may also be elevated.
2- Septic arthritis: infection in one part of the body, like the tonsils or the skin, travels through the bloodstream and affects one joint (the knee in this case) few weeks after this infection had resolved. Pain, rredness, and limited range of motion of the affected joint are observed. Elevated WBC, ESR and CRP are noted, and the X-ray is usually of limited value as it doesn't show the defect. Doing a joint tap (extraction of fluid from the joint) is essential tomake the ddiagnosis (identify if it's a bacteria, which necessitates antibiotics, or viral, so only anti-inflammatory medications are required).
